As soon as possible, we are looking for you as Case Manager (m/f/d) for our Helios Clinic at the Titisee-Neustadt location

• Conception, planning and implementation of nursing quality audits in defined areas of responsibility
• Implementation and execution of nursing rounds
• Patient Counseling and Patient Education
• Determination of joint treatment paths with medical colleagues and coordination of patient/family discussions
• Case management and coordination of the diagnostic and therapeutic components of the patients
• Coordination of discharge management and support of case coding

• Successfully completed training as a nurse (m/f/d)
• specialization, further education or studies in the field of nursing,
• Nursing science, quality management or comparable fields of competence
• High level of professional and social competence
• Distinctive, independent and structured way of working.
• Empathy, professional communication skills and enthusiasm
